Goal Counseling:
The Community SOAR project program model consists of comprehensive case management to address those barriers such as substance use, untreated mental health concerns, and lack of family support that negatively affect a person’s housing, transportation, and employment resources.
Case management support will allow program participants to find their strengths while minimizing those barriers. While in the program, they will get ongoing management and monitoring of their class attendance, appointment keeping, progressions and defaults. The case manager will activate community resource referrals and ensure that participants are moving towards reaching their personal, educational, financial, and community relationship goals.
Attentive and nurturing guidance is provided throughout the program encouraging participants to obtain stable housing, reliable transportation, gainful employment, child-care or family resources, and so much more.
Employment / Small Business Creation:
The Community SOAR Project recognizes the importance of employment or small business creation amongst its service population. Engaging in gainful employment has been shown to reduce the likelihood of becoming justice-involved, as well as, increase the likelihood of developing positive relationships and caring for one’s family. We support our Participants by assisting them to explore ideas, obtain certifications, acquire tools, or other needs while providing support and cognitive-behavioral life skills practice.
Our 24-Week Project Timeline:
WEEK 1.
Referral received by Project coordinator for input into database and scheduling of initial appointment. The coordinator will alert the project manager to the referral and scheduled appointment.
WEEK 2.
Appointment with Project Manager to conduct full intake and scheduling of assessments. Once all assessment information has been received the applicant is assigned a seat in the CBI: Thinking for a Change class.
WEEK 3 - 8.
Ongoing case management and monitoring of attendance to CBI class and scheduled appointments. Information is gathered related to housing, family and community support, transportation needs, etc. Community referrals will be activated at this point to ensure barriers are lessened prior to enrollment into the employment phase of the program.
WEEK 9.
Project manager meets with Supervisor to review case thus far. Upon successful recommendation, the applicant is referred to the Employment Manager of the program in which they have decided to participate.
WEEK 10 - 24.
Applicants are attending CBI and involved in their employment track. Some applicants may be attending Coastal Community College for their program of study. SOAR Project funds will be used to pay for short-term employment-related courses. During this period applicants are continuing to be evaluated for needs related to stable housing, transportation, and child-care.
